We do accept some insurance for Massage Therapy, please refer to the Insurance Information Sheet for more details on the insurance companies we accept.
*NOTE: This is a partial list of major insurances we accept or for which we are preferred providers. This list changes as we add insurances. Please do not hesitate to call to see if we accept your insurance provider. We also offer a cash discount for our cash pay patients.

Save time at your first appointment, your form can be filled out online and emailed back. The PDFs below works best with internet explorer or Microsoft edge web browser, if you don’t have Internet explorer or Microsoft Edge the instruction to properly save are below. Note that just pressing save and download will not save the forms.

If you need a doctor’s referral for physical therapy for your insurance you may ask your provider for a referral to wherever you would like to go. If you choose to come to Balanced Physical Therapy, we can accept a written order on any form or you may print this prescription form to bring to your doctor to sign.
